Job DetailsJob Location: Marathon - Marathon, WI 12345Position Type: Full TimeEducation Level: 4 Year DegreeSalary Range: $22.00 - $24.00 HourlyTravel Percentage: Up to 50% Job Shift: DayJob Category: Nonprofit - Social ServicesKey Responsibilities Safety & Crises Intervention Ensure the immediate safety of all individuals being assessed through active crisis management and de-escalation Administer the Columbia Suicide Severity Rating Scale (C-SSRS) to evaluate risk and determine appropriate interventions Develop and implement individualized Wellness Recovery Action Plans (WRAPs) and safety plans Provide gatekeeper training to empower Veterans, family members, and community partners in recognizing warning signs and responding effectively Assessment & Individualized Support Planning Conduct comprehensive mental health screenings and needs assessments to identify barriers and strengths Develop Individualized Service Plans (ISPs) that promote recovery, self-determination, and personal growth Monitor and document participants’ progress, adjusting goals and supports as needed to ensure continued engagement and improvement Case Management & Care Coordination Refer and connect participants to appropriate community and VA resources, including behavioral health, housing, and employment services Assist participants with scheduling and attending initial appointments; follow up to confirm participation and continued care Advocate for participants’ rights, ensuring they receive equitable, respectful, and trauma-informed services Participant Education & Skill Development Conduct one-on-one and small-group sessions on essential life and recovery skills, including: Suicide prevention and awareness Accessing and utilizing community resources Understanding participant rights Budgeting and financial literacy Daily living and coping skills Community Outreach & Collaboration Maintain and strengthen relationships with existing referral sources, including VA partners, local agencies, and Veteran service organizations Identify and cultivate new referral resources to expand the program’s reach and impact Represent the program at community events and collaborate with partners to promote awareness of suicide prevention resources Documentation & Compliance Maintain accurate, timely, and confidential documentation of all participant interactions in accordance with program and agency standards Ensure compliance with all policies, procedures, and federal regulations governing suicide prevention services Participate in staff meetings, training, and program evaluations to support continuous improvement Other duties as requested or assigned Key Performance Indicators 100% of participants assessed for suicide risk using the C-SSRS within 24 hours of referral 100% of identified at-risk individuals receive a completed WRAP or safety plan within 48 hours of assessment 100% of case notes, assessments, and ISPs entered accurately and within 48 hours of service delivery 90% of participants maintain engagement with at least one follow-up contact per month during the active service period 95% of referrals to external resources completed with documented follow-up within 7 days 85% of participants demonstrate measurable progress toward at least one ISP goal within 60 days QualificationsQualifications Minimum two years of experience in case management including needs assessment and service planning Bachelor’s degree in social services. Substitution of Associate degree with 4-5 years of experience in case management including needs assessment and service planning is acceptable Experience working with Veterans strongly preferred Experience delivering services to individuals with complex and intensive needs, including mental disorders and/or substance abuse, strongly preferred Resident of the proposed service area with broad familiarity with the region, its people, and its resources preferred Valid Wisconsin driver’s license in good standing and current/valid auto insurance
