Continuum Care Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) Case Manager to join our growing team. We are committed to providing exceptional end-of-life care that promotes comfort, dignity, and quality of life for our patients and their families.

Position Summary

The LPN Case Manager provides skilled nursing care to hospice patients in homes, assisted living communities, and skilled nursing facilities. Working as part of an interdisciplinary team, the LPN assists in implementing the patient's plan of care, monitors clinical status, provides education and support to caregivers, and helps ensure the highest quality of hospice services.

Responsibilities

  • Provide direct patient care in accordance with hospice policies and physician orders.
  • Monitor patient condition and report changes to the RN Case Manager and physician.
  • Administer medications and treatments within scope of practice.
  • Reinforce patient and caregiver education regarding disease progression, symptom management, and end-of-life care.
  • Collaborate with physicians, nurses, social workers, chaplains, hospice aides, and other team members.
  • Participate in care planning and interdisciplinary team meetings.
  • Maintain accurate and timely clinical documentation.
  • Support patients and families with compassion, dignity, and respect.

Qualifications

Required

  • Current Ohio LPN license in good standing.
  • Valid driver's license, reliable transportation, and proof of auto insurance.
  • Strong communication and organizational skills.
  • Ability to work independently while collaborating with a multidisciplinary team.

Preferred

  • Hospice, home health, long-term care, or geriatric nursing experience.
  • Electronic medical record (EMR) experience.
  • Knowledge of end-of-life care principles.
