 SCHEDULER 

 

GRADE: 5   Non-exempt    REPORTS TO:  PCA Supervisor/Home Care Coordinator

 

POSITION REQUIREMENTS: High School diploma, GED or equivalent, Associate degree 

will be considered. Previous administrative experience preferred, minimum two (2) year’s experience in the area of scheduling, transportation, human services, disabilities, or other related field is required; advanced knowledge of computers is recommended; must have demonstrated excellent interpersonal communication skills.  

 

SUMMARY: Engages and interacts with clients, Personal Care Aides, and Homemakers to coordinate and schedule appropriate services, according to a Plan of Care and regulations. Maintain database system of clients, Personal Care Aides, and Homemakers. 

 

DUTIES AND RESPONSIBILITIES:

 

1.         Schedule and coordinate Personal Care and Homemaker services with appropriate notification and documentation.

2.          Collaborate with Supervisor/Home Care Coordinator, and other IDT members to maintain a good working relationship. 

3.         Attend PACE morning meetings, IDT, or other trainings as assigned by Supervisor/Home Care Coordinator. Must always review PACE morning meeting minutes. 

4.         Demonstrate professional verbal and written communication.

5.         Demonstrate excellent computer skills.

6.         Demonstrate decision making and problem-solving skills.

7.         Recognize and report significant changes, abnormal observations, and incidents to Supervisor/Home Care Coordinator, PACE, or another appropriate source, such as emergency support.

8.         Maintain the highest degree of confidentiality in all aspects of client relations 

and Agency operations.

9.         Maintain database system of patients Personal Care Aides and Homemakers       containing schedules, aide records, Plan of Care, telephone numbers and directions, etc.

10. Comply with HIPAA requirements and will maintain strict 

             confidentiality with regards to clients and Agency operations. 

11.       Perform other duties as assigned.

  1. Advanced abilities to use hardware, software, and related technology to perform tasks. 
  2. Excellent interpersonal communication skills.
  3. Excellent analytical and problem-solving skills.
  4. Understanding of scheduling and resource allocation concepts.
  5. Knowledge of geographical area served by AASC.
  6. Ability to work independently with minimal supervision.
