JOB DESCRIPTION:
SUMMARY OF POSITION
This position provides High Fidelity Wraparound (Tier 1) and Intensive Care Coordination (Tier 2) services to youth and families with complex behavioral health and social needs. The role requires regular travel within the southern Illinois counties of Alexander, Pulaski, Union, Williamson, Jackson, Johnson, Massac, and Franklin and involves extensive community-based, in-person work. Travel is reimbursed in accordance with organizational policy.
The Behavioral Health Care Coordinator facilitates child and family team meetings, completes and reviews IM+CANS assessments, supports crisis services, and develops, monitors, and evaluates prevention and safety plans. This position works collaboratively with Pathways and other internal and external service teams to ensure coordinated, person-centered care. Responsibilities also include connecting clients to appropriate services, advocating on their behalf, providing ongoing support, and monitoring service effectiveness.
Essential Duties and Responsibilities
- Provide Care Coordination and support for youth and families receiving High Fidelity Wraparound (Tier 1) or Intensive Care Coordination (Tier 2) services.
- Work collaboratively with clients, family members, and multidisciplinary providers to develop, implement, and monitor individualized plans.
- Conduct in-person, community-based child and family team meetings in accordance with program and model requirements.
- Provide assertive outreach, advocacy, support, and follow-up to assigned clients; document all outreach attempts and ensure services outlined in the personal growth plan are delivered at the identified frequency.
- Provide ongoing case management services, including assessment, planning, linking, monitoring, referral, and advocacy.
- Deliver case management services to youth enrolled in the Family Support Program (FSP) and/or Specialized Family Support Program (SFSP).
- Assist clients and families in building and sustaining natural support teams, integrating community supports and resources across identified life domains.
- Incorporate natural and community-based supports into both the personal growth planning process and ongoing service delivery.
- Present relevant clinical and service information to ensure quality of care, obtain feedback, and guide service planning and adjustments.
- Identify, document, and report Critical Incidents in accordance with Centerstone policy and procedures.
- Support crisis planning and prevention efforts, including evaluation of safety and prevention plans.
- Provide community education and outreach activities as assigned.

- Knowledge of role and function of various community resources.
- Skill in problem-solving.
- Knowledge of basic service recipient’s dynamics of assigned disability groups.
QUALIFICATIONS
Education Level
Bachelor’s Degree in a human services-related field, OR
Associate’s Degree in human services plus at least three years of hands-on experience in mental health, OR
High School diploma or equivalent with a minimum of five years of supervised experience in behavioral health (subject to approval).
