Your EverydayĀ
- Coordinate written and verbal communication relating to the medical treatment plan between the patient, family, physician, physician staff, and other team members.Ā
- Communicate and coordinate the treatment plan to internal and external departments and agencies,Ā establishingĀ and maintaining the continuity of patient care.Ā
- AssistĀ patients in understanding their diagnosis, treatment options, and the resources available.Ā
- Review the medical documentation toĀ assistĀ the physicians in obtaining complete andĀ accurateĀ documentation of their medical treatment plan.Ā
- Serve as an essential link between patients and all other care providers.Ā
- Follow patients throughout the care continuum collaborating with care management resources andĀ facilitatingĀ appointments for consults and support services including social workers, PT, OT, ST, dietitians, physicians, financial resources, etc.Ā
- Collect data, track outcomes, and support strategic planning processes. Demonstrates the knowledge, skill, and coordination to provide nursing care and guidance from screening to survivorship by planning, implementing, and evaluatingĀ patientāsĀ care.Ā
- AssistĀ patients and their family members in search of solutions, treatment-related problems, education toĀ facilitateĀ seamless care and support patients withĀ difficult decisions.Ā
- Perform ongoing review of policies related to service provided whereĀ appropriateĀ and update policies to enhance professional practice.Ā
- Serve as a resource for community educational events such as health fairs, screenings, symposiums, and lectures as well as staff education.Ā
The Must-HavesĀ
Ā
Minimum:Ā
- Current nursing license to practice in Louisiana and as defined by the Louisiana State Board of Nursing.Ā
- AmercianĀ Heart Association BLS certification.Ā
- 3 years of professional nursing experience inĀ specialtyĀ area.Ā
