A Brief Overview

Participates, under supervision of residency preceptors, in the provision of comprehensive clinical pharmacy services and quality assurance programs. Rotates through an established program of education as assigned by the department/Residency Program Director. Performs pharmacist duties including compounding and dispensing medications, monitoring medication therapy, consulting and providing education to patients and healthcare professionals and monitoring data, inventory and quality metrics.

What you will do

  • Under assigned preceptorship interprets, processes, and monitors medication orders issued by providers for contraindications, interactions, allergies, and appropriateness to provide optimal patient medication resources in a timely manner.
  • Provides medication information to patients and health professionals and performs inventory control measures during medication distribution and administration for quality and safe patient care.
  • Counsels patients on the proper usage, desired effects, and potential adverse effects of medication.
  • Monitors medication therapy, including routine analysis of patients’ medication therapy, medical problems, laboratory data, and special procedures as well as correlating these analyses with other clinicians responsible for the patient’s care to provide a continuum of care throughout the hospital system.
  • Actively participates in the admission and discharge process to ensure patient satisfaction and understanding of medications.
  • Responsible for data collection and/or reporting for assigned drug utilization evaluations and quality assurance audits to ensure optimal medication use within the organization.
  • Prepares program content and presents educational presentations to health professionals to further develop hospital staff members and ensure professional advancement.
  • Performs functions related to clinical activities such as kinetics, medication therapy monitoring, medication information, and educational programs.

Essential Qualifications

Required Education, License/Certification, or Work Experience:

  • Doctorate (PHD) Pharmacy or
  • Doctorate (PHD) and completion of an accredited PGY1 residency for PGY2 residents
  • Pharmacist - Board of Pharmacy An active license is required within 45 days
