Hospice Social Worker (BSW or MSW)

We are looking for a compassionate, grounded Social Worker to support patients and families as they navigate some of life’s most important moments. This role is part of an interdisciplinary team focused on providing comfort, dignity, and support wherever patients call home.

What You’ll Do

  • Provide emotional and psychosocial support to patients and families 
  • Complete assessments and contribute to individualized care planning 
  • Assist with advance care planning and end-of-life decision support 
  • Connect patients and families with community resources 
  • Support with grief, coping, and caregiver stress 
  • Collaborate closely with nurses, aides, chaplains, and physicians 
  • Participate in IDG meetings and team-based care planning 


 What We’re Looking For

  • Bachelor’s or Master’s degree in Social Work (BSW or MSW) 
  • Current social work license (or eligible) in Virginia
  • At least 1 year of experience in healthcare, hospice, or a related human services setting 
  • Strong communication and relationship-building skills 
  • Ability to work independently while staying connected to a team 
  • Compassion, presence, and the ability to meet people where they are
