Full-time position available as a Permanent Supportive Housing Mental Health Case Manager. This position provides assessment, linkage, and advocacy services to individuals in the PSH program that wish to receive housing case management services that promote individual empowerment and reduce the need for more intensive services. Case manager will collaborate closely with individuals, families, and other service providers to develop and implement individual service plans focused on housing stability. This provider will be responsible for building rapport and maintaining positive, professional relationships with landlords in the community. Permanent Supportive Housing services will be provided to clients who are experiencing chronic homelessness, have been diagnosed with a Serious Mental Illness, are frequent users of emergency services, etc.. SOAR certified or applicants that are eligible to become certified are encouraged to apply. Preferred candidate will have Bachelor's degree in Human Services and meet criteria to qualify as a Qualified Mental Health Case Manager as defined by DMAS and DBHDS, demonstrated knowledge of area services, community partners, and resources, and one year of behavioral health direct care experience with individuals or families coping with mental illness, substance use or emotional disturbance.  Salary is commensurate with appropriate education and experience as it relates to the position.
