Schedule: 8:00 AM–4:30 PM | Holiday & Weekend Rotation | Field-Based

Nursing That Feels Different

Are you an RN who wants to spend less time fighting the clock and more time actually caring for your patients?

As a Hospice RN Care Manager, you’ll provide skilled nursing care to patients in their homes and care facilities throughout Marysville, Ohio, and the surrounding communities.

This is nursing with purpose. You’ll build relationships with patients and families, use your clinical judgment, work independently in the field, and have the support of an interdisciplinary team behind you.

You don't need hospice experience to make the move. If you're an experienced RN looking for a different kind of nursing career, we'll help you learn hospice.

What You'll Do

  • Provide compassionate, patient-centered nursing care in homes and care facilities.
  • Assess patients and families and develop individualized plans of care.
  • Monitor changes in condition and adjust care, medications, interventions, and visit frequency as needed.
  • Communicate changes in patient condition and care needs with physicians and the hospice team.
  • Coordinate care with nurses, physicians, social workers, aides, chaplains, volunteers, and other members of the interdisciplinary team.
  • Educate and support patients, families, and caregivers.
  • Advocate for patients and help families navigate the hospice experience.
  • Complete accurate, timely documentation in the electronic health record.
  • Participate in hospice admissions, IDT meetings, care conferences, and quality improvement activities.
  • Support and mentor team members as needed.

What We're Looking For

Required:

  • Graduate of an accredited nursing program.
  • Current RN license in the State of Ohio in good standing.
  • Current CPR certification.
  • Valid driver's license and safe driving record.
  • Required auto insurance.
  • Ability to travel throughout the assigned territory and safely drive in varying weather conditions.
  • Ability to complete required background check and drug screening.
  • Basic computer skills and accurate documentation ability.

Preferred:

  • Two years of acute care nursing experience.
  • Hospice or palliative care experience.
  • CHPN certification.

New to hospice? No problem. Certification preparation and support are available for eligible employees.
