As the Adoption & Permanency Support Caseworker you will be specialized in Supportive services to Adoption, Kinship and Sub Guardianship families. Works in a blended role capacity, providing permanency services to families pre- and post- adoption, support to subsidized-guardianship families, and aids in RRTS needs as required. RESPONSIBILITIES: Supports pre-adoptive, post-adoptive, kinship and sub guardianship families through to adoption or permanency and thereafter to assure stability and prevent disruption. Attends in-person warm hand off meetings with the RRTS Caseworker when arranged Develop support plans with families to identify needs, supports and crisis planning Provide crisis response in the home when families and children are needing immediate support to ensure disruption or child abuse does not occur Develop and maintain Adoptive Support Groups in collaboration with FOFC staff and other stakeholders Participate in frequent supervision, staff meetings and trainings as requested Meets contract requirements for adoption support referral process Provides Adoption related in-service training as requested Documents contact with families timely using appropriate CareMatch procedures and required forms May be asked to complete initial home studies as requested/needed May be asked to complete other RRTS contract work as requested/needed Demonstrates professional written and verbal communication with HHS/JCS and other stakeholders Travels as necessary to complete functions of job including attending stakeholder meetings, meeting families in their home environment, attending training or support group venues. Performs all other duties as assigned.   Qualifications & Program InformationWhat you need: BA Degree in Human Services, Social Work, Behavioral Sciences or related field, plus one year of related experience. Knowledge of the foster care and adoption systems is preferred.
