Seasoned MDS Coordinator (RN or LPN) – Full-Time
In-House Position | Massillon, OH

Are you an experienced MDS Coordinator looking for a stable, full-time opportunity where your expertise is valued? We are seeking a seasoned MDS professional to join our team in Massillon, Ohio. This is an on-site position designed for an MDS expert who can confidently lead the assessment process and support quality resident outcomes.

Please Note:
This is an in-house, full-time position. We are only considering experienced MDS Coordinators with a proven background in completing and managing the MDS process.

What You'll Do

  • Complete and oversee MDS assessments accurately and on time 
  • Coordinate Medicare and PPS assessments 
  • Ensure compliance with CMS and state regulations 
  • Drive Case Mix optimization and reimbursement accuracy 
  • Monitor Quality Measures and identify improvement opportunities 
  • Collaborate with nursing, therapy, dietary, and interdisciplinary teams 
  • Maintain accurate documentation and support survey readiness 
  • Stay current on MDS regulations and reimbursement changes 

Qualifications

  • Current RN or LPN license in Ohio 
  • Extensive experience as an MDS Coordinator in a skilled nursing or post-acute setting 
  • Strong knowledge of MDS 3.0, RAI process, PPS, PDPM, Case Mix, and Quality Measures 
  • Excellent assessment, organizational, and communication skills 
  • Ability to work independently while collaborating with the interdisciplinary team 
  • Experience with electronic medical records preferred
