Job Summary

Responsible for working closely with individuals and communities to promote and maintain good health. Responsible for providing education, support, and outreach services to community members to help them achieve their health goals. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. Works under the direction of a healthcare professional to provide preventive care and health education to members of the community.

Primary Responsibilities:

• Conduct community outreach and engage community members to understand their health needs.
• Provide individual and group health education and coaching to community members.
• Assist community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services.
• Provide basic health screenings and assessments to identify health risks and provide appropriate interventions.
• Assist in the development of health promotion programs and activities to address community health needs.
• Collect data and maintain accurate records of community health outreach activities and interventions.
• Attend meetings and trainings related to community health promotion and education.

Qualifications

Qualifications:

  • High School Diploma required. Associate's Degree or Bachelor's Degree in related field of study preferred.

  • Valid Driver's License and reliable form of transportation required.

  • 1+ years of experience in community outreach, health education or related field strongly preferred.


Additional Knowledge, Skills and Abilities:


- Strong communication and interpersonal skills, with the ability to interact effectively with various populations.
- Demonstrated ability to work effectively and provide advocacy for all populations and communities preferred.
- Ability to work independently and as part of a team.
- Basic computer skills, including Microsoft Office and database management.
- Bilingual skills preferred.

Schedule and Work Model:

  • Full-time, Monday through Friday, standard business hours.

  • Hybrid work model: Must be flexible to work in-office and/or travel in the community up to four days per week. Travel will be primarily within Acton, Concord, Maynard, Littleton, Westford and surrounding communities.

  • On remote workdays, employees must use a stable, secure, and compliant workstation in a quiet environment. Teams Video is required and must be accessed using MGB-provided equipment.
