About the Role:

The RN Home Health position is a critical role focused on delivering high-quality nursing care to patients within their homes across the United States. This role involves comprehensive assessment, planning, implementation, and evaluation of individualized care plans to promote patient health, manage chronic conditions, and support recovery. The nurse will collaborate closely with interdisciplinary teams, patients, and their families to ensure continuity of care and adherence to best practices and regulatory standards. The position requires adaptability to diverse home environments and the ability to provide compassionate, patient-centered care that respects individual needs and preferences. Ultimately, the RN Home Health professional plays a vital role in enhancing patient outcomes and quality of life while reducing hospital readmissions and healthcare costs.

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in the state of practice within the United States.
  • Graduation from an accredited nursing program (Associate's or Bachelor's degree in Nursing).
  • Basic Life Support (BLS) certification.
  • Minimum of one year of clinical nursing experience, preferably in home health, community health, or related settings.
  • Strong knowledge of nursing principles, patient care standards, and home health regulations.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN) degree.
  • Certification in Home Health Nursing (e.g., Certified Home Health Nurse - CHHN).
  • Experience with electronic health records (EHR) and telehealth technologies.
  • Advanced certifications such as Wound Care or Chronic Disease Management.
  • Demonstrated experience in patient education and interdisciplinary team collaboration.

Responsibilities:
