Responsibilities

  • Evaluate patients and develop personalized care plans focused on functional independence, safety, and quality of life
  • Deliver care through therapeutic activities, ADL training, and adaptive techniques
  • Help patients improve their ability to perform daily tasks such as dressing, bathing, grooming, and meal preparation
  • Partner with interdisciplinary teams, caregivers, and nursing staff to support strong patient outcomes and continuity of care
  • Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals

Qualifications:

  • Licensed Occupational Therapist (OT) or license eligible in the state of practice
  • Graduate of an accredited Occupational Therapist Program
  • Interest or experience in skilled nursing, long-term care or geriatric rehabilitation
