Job DetailsLevel: ManagementJob Location: The Cedars - McPherson, KS 67460Position Type: Full TimeEducation Level: Nurse LicenseTravel Percentage: MinimalJob Shift: DayJob Category: Health CareThe Cedars is looking for a full-time LPN or RN Clinical Care Coordinator to join our team!  Position Overview: The primary responsibility of the Clinical Nurse Manager is to assist the Director of Nursing to direct and coordinate resident care activities, maintain standards of resident care, and advise medical staff, department heads, and administrators in matters related to nursing services. These tasks will be performed by the Clinical Nurse Manager or may be delegated to subordinate supervisors. The Clinical Nurse Manager will direct and coordinate the skin and wound program for the facility and coordinate focused training and competency checks for nursing staff.  Essential Nursing Functions: In the absence or at the request of the Director of Nursing Services, conduct staff meetings Assist with the facility training initiatives for all staff, to include general healthcare orientation, required in-service training, clinical training for the nursing department, and staff competency skill fairs Advise and assist Director of Nursing, and medical staff in planning and developing nursing administrative and operational policies and procedures, facilities use and planning, and strategic planning for the nursing department Confer with managers and staff concerning problems of resident care Make daily rounds on all resident units Chart reviews (per BS chart review) Care plan initiation/updates, including 48 hours care plan Wound report—fixing errors in Module, making sure the monitoring/dressing changes are on the MAR, checking documentation, going and looking at wounds as needed Supply coordination w/Melanie for all nursing and specialized supplies Daily Lab/imaging review  IV – orders, supplies, contact person for Infusion Narcotic/Antipsychotic tracking included informed consents per state regulations Hospice contact, Wound care referral and contact Policy and Procedure On Call SLAT – review, consider, screen prospects Review and Revision of monthly LN and CNA training and skills – complete monthly training in collaboration with RISK/IPC nurse Scheduling check ins, LN interviews, help with other interviews and counseling’s when needed NOC to DAY check-in w/nursing Assisting with admissions- Checking orders, meds, communication with family/nurse Check each nurse’s notebooks and make sure faxes are getting returned. Making sure the skin concerns are being entered and monitored in PCC correctly and ensuring weekly skin checks are completed Checking JANA and follow up with BM list, No bathing list, low nutrition list and active alerts Review of weekly vital signs including weights to ensure that any out of range items have been addressed Contact with residents for any complaints that charge nurse is unable to take care of Communicating with nurses as errors or issues arise (ie: new nurse’s not making packets correctly) and corrective actions when needed Run the “Order Listing Report” and follow-up on documentation as needed Check BGM Controls weekly, and follow-up as needed Ensuring LICAMEDMAN is completed Attend daily clinical rounds, weekly risk management, weekly survey ready and monthly QAPI meetings Participate in state surveys as directed by DON/Administrator Qualifications and Experience: Must be at least eighteen [18] years of age Must be a Registered Nurse licensed to practice in Kansas, BSN required Must have three years of responsible nursing experience, preferably in a long-term care facility Must be knowledgeable of State and Federal regulations for nursing homes Must have strong management and leadership skills Must have the ability to relate with different workplace populations Must be responsible, accountable and able to cope with diversity Prefer one (1) year experience with wound care management  Physical Requirements: Medium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly.
