We are seeking a warm, dedicated School-Based Case Manager to support elementary through high school students within the Smoky Valley School District (USD 400). In this role, you’ll serve as a vital link between students, families, educators, and mental health resources—helping young people build resilience, overcome challenges, and succeed both in and out of the classroom. Key Responsibilities Student & Family Support: Deliver strengths-based, community-based case management in school settings. Help students build self-esteem, problem-solving abilities, and healthy coping mechanisms. Skill Building & Advocacy: Teach essential life and behavioral skills, empower parents with effective strategies, and connect families to vital community resources. School & Team Collaboration: Work closely with school counselors, teachers, and outpatient clinical staff to support treatment plans and coordinate care. Crisis Intervention: Provide calm, supportive crisis intervention when students experience acute distress, ensuring personal and environmental safety. Documentation & Compliance: Maintain timely, accurate clinical progress notes and service documentation within agency guidelines.   Qualifications Minimum Education: Either 4 years of equivalent work experience or bachelor’s degree Minimum Experience: Previous case management experience preferred.
