VNA Alliance is a faith based, non-profit organization based in Meadville, Pennsylvania. VNA Alliance serves Crawford, Mercer, Venango, Erie, Forest, Warren, Butler, and Lawrence counties. We offer skilled nursing, palliative care, and comprehensive therapy services.Â
The team assistant assists the case manager in coordinating, monitoring and evaluating options and services needed to meet a person’s health needs with the clinical field staff.
QUALIFICATIONS
Knowledge of Medicare homecare regulations
High school education required
At least one [1] year experience in a health care environment
Computer skills: Word and Excel Spreadsheets
Excellent organizational skills with attention to detail
Good communication and interpersonal skills
Ability to prioritize and organize daily workload
RESPONSIBILITIES
A. Reviews all incoming paperwork for documentation, compliance and completeness
B. Receives referrals from intake and disperses them to the appropriate case manager
C. Verifies daily visit records with schedules and visit reports for statistical and payroll purposes
D. Verifies visits and authorizations for disciplines via the computer on a weekly basis
E. Assists in preparing case management reports of all patients monthly for the PPS meeting
F. Alphabetizes paperwork and files appropriately
G. Tracks all insurance authorization visits and enters the information into the 3M system per agency protocol
H. Assists in gathering relevant statistical data
I. Assists in maintaining schedules for all staff
J. Enters patient information in the Navinet system
K. Provide coverage to other team case managers when a team assistant is not available due to vacations, etc.
L. Other duties as assigned
COMMUNICATION
Communicates all changes in the daily schedule to the appropriate supervisor and/or staff member
Notifies all disciplines involved in patient’s care of transfers and discharges
Identifies problems/concerns with patients and staff and reports them appropriately
Handles general inquiries whether received by telephone, mail, or in person to and from staff, patients, and referral sources
Attends weekly team meetings
Maintains the visit and authorization log and reports to the case manager on a weekly basis
Maintains ongoing liaison and communication with staff

Complies with risk management practices
Maintains confidentiality of patient and staff information
Working knowledge of agency policies, federal and state regulations and third party payers
Participates in Utilization and Quality Improvement activities
