What You’ll Do Lead and support the Neighborhood Care Team Administer medications and treatments accurately Assess and monitor resident health; communicate changes promptly Delegate daily assignments and ensure quality, person‑centered care Mentor CNAs, PCAs, and team members Collaborate on individualized care plans Ensure regulatory and HIPAA compliance Work a flexible schedule, including weekends/holidays What You Need Graduate of an approved LPN program (RN candidates welcome) Active, unencumbered PA LPN license Prior experience in Gerontology or Long Term Care preferred.
