As a RN Case Manager You will:   Â
- Assess/monitor physical, emotional, and psychological needs of patientsÂ
- Create hospice care plans that align with the patient's wishes and goals  Â
- Direct nursing care: administering medications, treatments, and interventions  Â
- Provide pain and symptom management Â
- Educate and support the patient’s family and caregiversÂ
- Collaborate with interdisciplinary teamÂ
- Maintain accurate and timely documentation Â
- Participate in on-call rotation as required by local branch  Â

Qualifications
-
- Graduate of an accredited nursing school with current licensure to practice in state of operationÂ
- One year nursing experience required, 3+ years preferredÂ
- Experience in hospice or a similar setting preferredÂ
- Knowledge of the hospice philosophy of careÂ
- Commitment to clinical and documentation excellenceÂ
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working orderÂ
