As a Nurse Practitioner You will:  Â
- Provide compassionate medical care and symptom managementÂ
- Perform assessments, order/ interpret diagnostic tests, and develop care plansÂ
- Prescribe medications, manage treatments, and coordinate hospice servicesÂ
- Conduct and document face-to-face visits for eligibility and recertificationÂ
- Collaborate with physicians and IDT team to align care with patient goals.Â
- Participate in care planning and coordinate with team and external providersÂ
- Maintain accurate clinical documentation and ensure compliance regulationÂ
- Educate patients & families on medical, emotional, and spiritual needsÂ
- Contribute to quality improvement efforts and professional developmentÂ
- Serve as a clinical resource and liaison to community partners

Qualifications
- Graduate of an accredited School of Nursing and Nurse Practitioner programÂ
- Master’s degree with a minimum of one year NP experienceÂ
- One year of NP experience or one year of RN hospice experienceÂ
- Minimum of one (1) year hospice or palliative care experience preferredÂ
- Current nursing license in the state of practiceÂ
- Current DEA license (if applicable to state regulations)Â
- Current CPR certificationÂ
- Certification from an approved advanced practice nursing certifying body, as required by the stateÂ
- Nurse Practitioner Certification in Gerontology, Adult, or Family (preferred)Â
- Excellent observation, verbal, and written communication skillsÂ
- Strong problem-solving and mathematical skillsÂ
- Proficient in nursing skills as outlined in the competency checklistÂ
- Demonstrated ability to counsel and communicate effectively with patients and families regarding end-of-life issuesÂ
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order Â
