Position Overview:

We are seeking a highly motivated and experienced Director of Social Services with experience working in a Skilled Nursing Facility to lead and oversee our social service programs. The successful candidate will be responsible for planning, developing, organizing, implementing, evaluating, and directing social service initiatives in accordance with current federal, state, and local standards, guidelines, and facility policies and procedures. This role ensures that the medically related emotional and social needs of each resident are met and maintained on an individual basis.

  • Lead and assist in the planning, development, organization, implementation, evaluation, and direction of the facility's social service programs.

  • Actively participate in discharge planning, and the development and implementation of social care plans and resident assessments.

  • Conduct interviews with residents and their families as needed.

  • Maintain accurate and timely Social Service progress notes, documenting responses to treatment plans and adjustments to institutional life.

  • Facilitate access to community social, health, and welfare agency resources to meet resident needs.

  • Maintain consistent contact with residents' families, providing non-medical progress reports as necessary.

  • Assist in the effective resolution of grievances voiced by residents, families, responsible parties, or ombudsmen.

Qualifications:

  • Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human service field (e.g., sociology, special education, rehabilitation counseling, psychology).

  • A minimum of one year of supervised social work experience, with a strong preference for experience in a skilled nursing setting.
