Position Overview:
TheĀ MDS CoordinatorĀ assumes full time administrative authority, responsibility and accountability to coordinate the completion of the required minimum data set and with other members of the inter-disciplinary team, develops and implements a plan of care that meets the individual needs of each resident.
Oversee the completion of MDS assessments across multiple facilities
Ensure compliance with federal and state regulations regarding MDS documentation
Provide training and support to facility staff on MDS processes
Monitor and analyze MDS data for quality assurance
Collaborate with interdisciplinary teams to develop and implement care plans
Facilitate regular meetings to discuss MDS updates and best practices
Respond to inquiries and resolve issues related to MDS documentation
Qualifications:
Bachelor's or Associate's degree in Nursing
LPN or RN license in the state of Tennessee
A minimum of 2 years of experience in skilled nursing facility or healthcare coordination
Strong knowledge of the MDS process and related regulations
Excellent communication and interpersonal skills
Proficiency in data analysis and reporting
Ability to work independently and manage multiple projects simultaneously
Certification in MDS (such as MDS Coordinator) is preferred
CMI (Case Mix Index) experience is a Must!
