Job DetailsJob Location: HVIP - Memphis, TN 38103Position Type: Full TimeTravel Percentage: NegligibleJob Shift: DayJob Category: Community Outreach JOB SUMMARY This Lead Violence Intervention Liaison is responsible for providing case management support to persons enrolled in the Hospital Violence Intervention Program (HVIP). The Lead Violence Intervention Liaison will provide pro-active mentoring, peer interventions, guidance and one-to-one advocacy to at risk youth and adult victims of gun violence, knife stab wounds and/or assaults.. This position will conduct in-hospital and client visits and work collaboratively with community partners and resources.     Key Job Responsibilities       Essential Function Supervises the Violence Intervention Liaisons daily work and schedules to ensure optimal program availability. Attends internal and external trauma meetings as requested to represent and report for the Hospital Violence Intervention Program. Suggests initial and ongoing training for the Hospital Violence Intervention Program. Prepares/presents reports as requested for Trauma Administration leadership team. Provides interventions based on defined standards of practice, counseling and advocacy to at risk youth and adults (and their families) hospitalized as a result of gun violence, knife stab wounds and/or assaults by understanding and relating to the needs of youth and adults of diverse ethnic, cultural, educational and socioeconomic backgrounds. Develop client service plans and provide intensive case management support which includes initial screens and client visits with crime victims during their hospitalization as well as after discharge and assistance in securing social services, education and job-related opportunities by assessing needs and providing information about services. Form positive relationships and maintain contact with program enrollees during and after their hospital stay. Work effectively with high-risk populations and network effectively with various types of organizations, including government agencies, law enforcement agencies, schools, social service agencies, courts, probation, corrections and grassroots organizations and hospital staff and physicians. Must be able to show effective client goal management and relationship development. Work with hospital staff (social workers, patient advocates, etc.) to help ensure enrolled clients have access to appropriate support services. Actively listens for root origins of shootings/killings to assist in mediating situations and preventing retaliation between individuals and groups (e.g., by working with community leaders, other outreach programs to share information that may be helpful in preventing retaliation and/or repeat intentional injuries. Regularly analyze trends to report opportunities for program/process improvement.   Essential Function   Coordinate activities, prioritize tasks and work independently. Exercises judgment regarding information sharing to abide by HIPAA and hospital confidentiality guidelines. Assists community organizations in the identification, design and implementation of violence intervention strategies and initiatives and participates in violence prevention efforts with other providers as assigned. Participates and represents Regional One Health with other programs and organizations working to prevent and reduce violence in the targeted populations.
