Pharmacists will perform comprehensive medication management and patient-focused pharmacy services in a high‑touch, ambulatory environment. This role supports pharmacy operations across meds‑to‑beds, home delivery, internal medicine clinics, and specialty care settings. The pharmacist works closely with providers, clinical pharmacists, care teams, and patients to optimize therapy, improve adherence, and ensure safe, effective medication use. Key Responsibilities Medication Verification & Dispensing Review, interpret, and verify prescriptions for accuracy, safety, and clinical appropriateness. Identify and resolve drug interactions, contraindications, duplication, allergies, and dosing concerns. Ensure accurate dispensing of medications and proper patient labeling in compliance with state and federal regulations. Oversee and check pharmacy technician work to ensure accuracy and quality. Patient Engagement & Adherence Support Provide high‑touch, personalized counseling on new and existing medications. Support medication adherence through proactive outreach, refill coordination, and follow‑up. Educate patients on chronic disease management, specialty therapies, and medication administration. Participate in meds‑to‑beds counseling to ensure smooth transition from inpatient to outpatient care. Outpatient Operations & Service Excellence Support pharmacy services including retail dispensing, bedside delivery programs, and home delivery. Ensure outstanding customer service and patient experience through timely, professional communication. Maintain accurate patient profiles, documentation, and electronic record updates. Adhere to all compliance, regulatory, and accreditation requirements (e.g., HIPAA, Board of Pharmacy, URAC/ACHC, Joint Commission). Assist in inventory management, including ordering, storage, and monitoring for expiration and integrity. Professional Practice & Continuous Improvement Participate in quality improvement initiatives and contribute ideas for enhancing pharmacy services. Maintain professional licensure and required continuing education. Participate in staff training, new-hire orientation, and precepting of students/residents when applicable. Stay current with medication therapy developments, specialty drug trends, and ambulatory pharmacy best practices. Qualifications Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy degree required. Active Pharmacist license in good standing. Outpatient, retail, or ambulatory care experience preferred. Strong communication, clinical judgment, and patient‑engagement skills. Ability to work collaboratively within an interdisciplinary healthcare team. Comfortable in a dynamic environment involving meds‑to‑beds, home delivery, and clinic‑support services.
