We are seeking a skilled and compassionate Physical Therapist to join Harborview Post Acute in Memphis. This permanent position focuses on working with post-acute patients recovering from surgery, illness, injury, and medically complex conditions. You will play a key role in helping patients regain mobility, improve strength and endurance, and achieve greater functional independence. Job Responsibilities:
• Evaluate, diagnose, and treat patients with physical impairments in a post-acute setting
• Develop and implement individualized treatment plans based on patient needs and clinical findings
• Collaborate with interdisciplinary teams to ensure coordinated patient care and recovery planning
• Supervise and work closely with Physical Therapist Assistants as needed
• Monitor patient progress and modify treatment plans based on clinical outcomes
• Educate patients and caregivers on mobility, safety, and home exercise programs
• Maintain accurate and timely documentation in compliance with facility and regulatory standards




Requirements

• Active state license as a Physical Therapist in Tennessee
• Current BLS/CPR certification
• Minimum of 1 year of recent PT experience preferred, ideally in post-acute, SNF, or rehabilitation settings
• Strong clinical reasoning and patient care skills
• Ability to work collaboratively within a multidisciplinary healthcare team
