Overview

Crossroads Hospice is looking for a self-motivated and passionate RN Hospice Hospital Liaison to join our Team. We offer the opportunity to take on new challenges and a culture of teamwork that sets us apart.

 

The RN Hospice Hospital Liaison will work with referral partners within local hospitals to identify and facilitate admissions of patients eligible for GIP level of care in the hospital, manage patient discharge from the hospital; and the admission process from the hospital to home or long-term care facilities. RN Hospice Hospital Liaisons use their clinical knowledge, empathetic communication skills, and marketing abilities to initiate one-on-one dialog with our hospital referral sources, families, and prospective patients to help explain and educate on hospice services. Our RN Hospice Hospital Liaison are hospice experts and serve as a resource to their referral sources to ensure extraordinary patient and customer service.

 

Schedule: Friday - Sunday, 12 Hour Shifts

Responsibilities

  • Works with referral partners within the hospital to identify and facilitate admissions of patients eligible for GIP level of care.
  • Manages patient discharge from the hospital; and the admission process from the hospital to home or long-term care facilities
  • Performs evaluation of patients for GIP level of care. Communicates with MDs, Discharge Planners, and/or Social Workers at hospital with disposition as well as liaison report if patient does not meet eligibility.
  • Promotes and positions Hospice's patient care service through regular visits to hospital partners in a manner that illustrates and reflects the Hospice's quality of care and unique capabilities.
  • Works to build and maintain Crossroads Hospice as the preferred hospice provider in our service area to achieve and exceed agency goals for growth and census management.
  • Conducts initial meeting with the patient and family giving an overview of hospice service and philosophy and obtains consents for care.

Qualifications

  • Bachelor's degree in nursing or health care preferred.
  • Holds current unencumbered license as a registered nurse.Holds current unencumbered license as a registered nurse.
  • Strong medical assessment skills; prior hospital, ICU and/or LTACH experience is required. Must possess strong oral and written communications skills.
  • Must have a current driver's license and be able to travel to hospital partners in the area.
  • CMS requires that all patient-facing staff, as well as those that are in contact with patient-facing staff, be fully vaccinated, or have a documented religious or medical exemption. This important rule is required for all healthcare organizations that receive federal funds through Medicare or Medicaid.

$85,000 - $95,000 per year (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
