Liquid Personnel is currently recruiting for an Adult Social Worker to join a Hospital Team based in Merton.

What will your responsibilities be?

• Conducting psychosocial assessments and developing care plans.
• Providing counselling and emotional support to patients and families.
• Coordinating with healthcare professionals to ensure comprehensive patient care.
• Assisting with discharge planning and connecting patients to community resources.

Qualifications and Experience:

To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
