  • Job Scope:
    The Resident Care Coordinator works as a liaison between residents, resident families, and staff. The
    RCC's duties also include problem solving resident concerns and coordinating care with the
    Caregivers. The RCC may be an RN or LPN when necessary.
    Responsibilities:
  • Responsible for caregiver scheduling and resident assignments, in compliance with community
    staffing guidelines.
  • Coordinate admissions including supervising move-ins to be sure accommodations are as
    desired and care is immediately implemented.
  • Conduct family and resident admission interviews.
  • Immediately bring prohibited conditions or at-risk residents in need of a higher level of care to
    the attention of the Administrator.
  • Coordinate care with home health agencies on site, working within community policy.
  • Coordinate RN Delegated task education and reviews with the corporate RN.
  • Arrange for transportation as desired by the residents.
  • Arrange for resident special needs involving other departments and verifies completion.
  • Function as a liaison with families ensuring special needs/requests/complaints are
    addressed. Inform Administrator, in writing and verbally, of all family or resident complaints.
  • Monitor staff performance, providing or arranging assistance as needed.
  • Review communication notes regarding the community between the Caregiver shifts.
  • Coordinate staff training and in-service schedules with the Administrator.
  • Supervise the medication room and orders, working with and supervising Medication Aides and
    Caregivers.
  • Foster teamwork with co-workers, show initiative in overcoming obstacles, take actions that
    assist others beyond job responsibilities, and have a positive impact on co-workers, residents,
    and visitors.
  • Seek out and perform other duties as assigned or needed.

Qualifications:
Two years of experience as an LPN/RN in senior care setting. Experience in Assisted Living Memory Care preferred.
Eligible to participate in federally funded health care programs.
Must be currently licensed and maintain state requirements for relevant licensures or
certifications and have no disciplinary action in effect against professional license.
