The Patient Intake Coordinator plays a critical role in ensuring timely, accurate, and efficient processing of patient referrals. This position serves as the first operational touchpoint in the patient journey and is responsible for coordinating referral intake, verifying eligibility, and coordinating with Nursing Management in an assigned area for seamless patient transitions.

The Patient Intake Coordinator works collaboratively with Nursing Management, Sales, Scheduling, and Quality to ensure exceptional customer service and adherence to operational standards.

Key Responsibilities

  • Process assigned incoming referrals within established timeframes (Send for Clinical Review within 30 minutes of receipt)
  • Referral assignment includes Portals, Web, Email, Faxes.
  • Verify patient demographics, coverage area, and insurance eligibility.
  • Communicate with hospitals and healthcare facilities to obtain any further documentation needed for speedy admission.
  • Confirm payer requirements and authorization needs.
  • Check internal registry and compliance lists (e.g., safety flags).
  • Upload and organize referral documentation in the electronic intake system (Enzo/Healthwyse) and maintain accuracy of data.
  • Coordinate with Nursing Managers for clinical review and start-of-care determination.
  • Provide professional, responsive communication to referral sources including hospitals, and providers.
  • Ensure a seamless experience for callers and external partners.
  • Educate referral sources on required documentation and referral processes.
  • Escalate urgent issues appropriately.
  • Adhere to established intake workflows and evolving process improvements.
  • Monitor intake queues and ensure timely movement through workflow stages.
  • Maintain up-to-date coverage area and territory knowledge.
  • Partner with Sales Account Executives to support referral relationships.
  • Work closely with Nursing Managers to align staffing capacity and service eligibility.
  • Participate in team meetings and process improvement initiatives.
  • Maintain HIPAA Compliance at all times.
  • Occasional office travel (Likely 2x per year) for company-wide meetings.

Qualifications

  • High school diploma required; Associate's or Bachelor's degree preferred
  • 1–3 years experience in healthcare intake, admissions, medical office, or related field
  • Knowledge of insurance verification processes (MassHealth/Medicaid, Medicare, commercial plans preferred)
  • Experience with Electronic Medical Records (EMR) or intake management software preferred
  • Strong organizational and multitasking skills
  • Excellent verbal and written communication skills
  • Excellent organizational & time management skills
  • Ability to work in a fast-paced, team-based environment
