The Care Coordinator is responsible for supporting patients and families by coordinating healthcare services, ensuring smooth transitions of care, and promoting adherence to treatment plans. Working across multiple practice sites, this role focuses on care management and utilization/quality improvement, medication refill management, coordination of care for special and post-acute populations, and tracking of key performance and accreditation metrics, in close collaboration with providers, payers, and community resources.

ESSENTIAL FUNCTIONS:聽
Care聽Management
路聽聽聽聽聽聽聽聽 Monitor patient progress and address barriers to care.
路聽聽聽聽聽聽聽聽 Facilitate communication between patients, clinical teams, and external partners.
路聽聽聽聽聽聽聽聽 Support utilization management and quality improvement activities.
Documentation & Compliance
路聽聽聽聽聽聽聽聽 Maintain accurate records in care management systems and EHR.
路聽聽聽聽聽聽聽聽 Ensure compliance with organizational policies and regulatory requirements.
路聽聽聽聽聽聽聽聽 Manage medication refill requests and processing in accordance with clinical protocols.
Patient Support聽& Navigation
路聽聽聽聽聽聽聽聽 Assist patient鈥檚 parents聽in understanding their care plans and available resources.
路聽聽聽聽聽聽聽聽 Coordinate appointments, referrals, and follow-up care across multiple providers.
路聽聽聽聽聽聽聽聽 Provide focused care coordination for special populations and post-acute care needs.
Resource Coordination
路聽聽聽聽聽聽聽聽 Connect patients with community services, financial assistance, and support programs.
路聽聽聽聽聽聽聽聽 Educate patients on preventive care and wellness initiatives.
路聽聽聽聽聽聽聽聽 Track and report on metrics related to accreditation, benchmarking, and progress toward program goals.
KNOWLEDGE, SKILLS & ABILITIES

EDUCATION & EXPERIENCE
- Bachelor鈥檚 degree in聽nursing, Licensed Practical Nurse certified, or a related field is preferred
- Minimum of聽1-2 years of experience in聽care coordination or case management and聽or patient services preferred.
