The University of Miami UHealth System Department of Site Disease has an exciting opportunity for a per diem Patient Navigator to work at Sylvester Support Service Building in Miami. The incumbent facilitates patients’ ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers.

CORE JOB FUNCTIONS

  • Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan.

  • Ensures patients have access to insurance and social service programs for which they are eligible.

  • Provides patient reminders of upcoming appointments and acts as liaison between patients and providers.

  • Identifies and resolves real or perceived barriers to care.

  • Educates and counsels patients on issues related to their health and life situations.

  • Maintains current knowledge of community health, welfare resources, and any other programs available to patients.

  • Handle patient communications and contact healthcare facilities for medical records.

  • Address care gaps with the nurse navigator.

  • Prepares, tracks, and processes medical records and pathology specimens. Reviews and file medical records for completeness; use tracking systems for accurate documentation.

  • Provides administrative support to the department by answering phones, address inquiries, and direct calls or take messages, Copy, fax, emailing, and mailing records while ensuring confidentiality.

  • Adheres to University and unit-level policies and procedures and safeguards University assets.

Education:
High School diploma or equivalent required

Experience:
Minimum 1 year of relevant experience required

Certification and Licensing:
