PSYCHCHIATRIC-​​MENTAL HEALTH NURSE PRACTITIONER

  Position Summary:

Under the general supervision of the Behavioral Health Services Director (BHSD), the Psychiatric-Mental Health Nurse Practitioner (PMHNP) will play an integral role in the Lazarus Project, providing compassionate, trauma-informed psychiatric and behavioral health care to individuals experiencing homelessness and other vulnerable populations served through Camillus House and community partners. We are seeking a passionate, mission-driven provider who is committed to meeting patients where they are and making a meaningful impact in their journey toward improved mental health, stability, and overall we​ll-being. The PNP will assess and diagnose mental health conditions, develop individualized treatment plans, provide medication management, monitor patient progress, and collaborate closely with an interdisciplinary team to ensure comprehensive, patient​-centered care in alignment with Camillus Health Concern’s mission, policies, and standards of care.


  Responsibilities:

  • Comply with the Mission and work in the Core Values of Saint John of God. 
  • Communicate on an ongoing basis with other health care providers and other staff on all aspects of the health needs of patients.
  • Complete all necess​​ary paperwork connected with duties, including sealing of progress notes in the Electronic Health Record in accordance with policy set forth by CHC. 
  • Ensures fiduciary responsibility by dropping charges for each encounter in accordance with the regulations set forth by CHC
  • Coordinate patient care with the Behavioral Health Team, clinical services, the Camillus House Institute of Personal and Social Adjustment (ISPA), and community agencies. 
  • Participate in and promote all quality improvement related activities as delineated in

CHC’s QI plan.

  • Ensure integration of behavioral health and psychiatric care with primary health care. 
  • Implement patient self-management tools. 
  • Participate in Clinical Case conferences. 
  • Be an advocate for clients within Camillus and the community.
  • Work effectively and professionally with Camillus staff and community partners. 
  • Meet appropriate biennial requirements for licensing and continued education as per the Florida Board of Nursing. 
  • Perform any other department or age​​ncy related duties or special projects as directed by Supervisor.


  Requirements:

  • Valid APRN license to practice as a Psychiatric Nurse Practitioner in the State of Florida
  • Eligible for Medicaid/Medicare provider status
  • Experience with public health issues as it relates to health center needs 
  • Strong interpersonal and communication skills
  • Strong organizational skills and documentation
  • Commitment to the mission of Camillus Healt​​h Concern, Inc.


  Preferred Qualifications:

  • Bilingual (English/Spanish), Creole a plus
  • Work effectively and productively in a team environment
    • Computer literate; previous experience with Electronic Medical Records
    • Culturally sensitive to the social, psychological and health care needs of the h​omeless and/or underserved populations
