POSITION SUMMARY

The Home Health Nurse Residency Program is designed to facilitate learning, support professional development, and socialize the newly graduated nurse to the role of a professional nurse. The Home Health Nurse Resident coordinates and directs the patient’s care based on individual patient needs under the direction of the clinical preceptor, clinical education, supervisor, and other clinicians. This role is designed to teach management of the Home Health patient population requiring the use of teaching, clinical decision-making skills, effective assessment and intervention, communication, critical thinking, and patient centered care. Work schedules are determined to meet patient care needs and work may be required on weekends and holidays.


ESSENTIAL JOB KNOWLEDGE

  • Establishes individualized, measurable goals in consultation with the patient, family, and other health care providers.
  • Develops a working knowledge of community resources and refers patients and families appropriately.
  • Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
  • Develops technical skills according to agency standards, as measured by competency assessments, during orientation and annually.
  • Documents components of the nursing process to reflect a comprehensive and integrated approach to nursing care.
  • Recognizes and appropriately reports potential problems and sets priorities according to changes in the home health patient’s condition.
  • Exhibits sound nursing judgment and decision-making skills in coordinating patient care.
  • Participates in and assists in the ongoing development of home health policies, procedures, standards of care and documentation systems.
  • Addresses concerns or complaints voiced by patients, families, caregivers, or other internal and external customers and notifies supervisor.
  • Identifies ethical conflicts and able to identify resources and options for addressing ethical issues and usual agency patterns, including involvement of the supervisor, primary care provider, and social services.
  • Involves the patient and family in the plan of care, incorporating their cultural, spiritual, and other belief systems.
  • Successfully completes initial and ongoing organizational and department education, training, and competency requirements.
  • Infection Control: Understands, demonstrates, and follows organizational infection control guidelines (CDIC).
  • Quality: Focuses on improving processes and champions efficiency in daily work activities.
  • Safety: Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization’s occurrence reporting system and participating in developing improved safety processes, as appropriate. (ADM015, ADM016, OPS044, Mission Statement).
  • Performs other duties as assigned.


JOB REQUIREMENTS 

Licenses:

  • State of Wisconsin Driver’s License is required.
  • State of Wisconsin Professional licensure within the past 12 months for Registered Nurse is required. 

Education:

  • Associate degree in: Registered Nursing required.
  • Bachelor’s degree in: Registered Nursing preferred.

Experience:

  • Experience with home health Electronic health Record (EHR) is preferred.
  • Experience and strong knowledge base in patient facing nursing care is required. 

Miscellaneous:

  • On-call when transitioning to RN case manager is required. 
  • CPR/BLS certification is required
  • Valid Auto Insurance is required.
  • Reliable vehicle transportation is required.
