POSITION SUMMARY

The Home Health Agency (HHA) Manager is responsible for operational, financial, programmatic, and personnel activities for the home health agency. The Manager provides direction and leadership to meet strategic and operational objectives. The Manager is the acting Administrator per DHS 133.06(1).


ESSENTIAL JOB KNOWLEDGE

  • Fosters a culture of learning, collaboration, and professional practice consistent with the mission, vision, and values of UW Health Care Direct (UWHCD).
  • Directs operations and resources to deliver efficient and effective patient care, in accordance with organizational and program strategy, goals, and objectives.
  • Effectively plans, implements, and manages operations and projects; articulates and presents data, information, and ideas clearly and concisely.
  • Evaluates program outcomes; drives process improvement discovery, solution development, and implementation via improvement science, methodologies, and tools.
  • Leads the home health agency Quality Assurance and Performance Improvement (QAPI) program.
  • Maintains home health industry knowledge and expertise.
  • Understands and adheres to all applicable state, local and federal laws and/or regulations, company policies and procedures, and external accreditation/review standards; assures continuous survey readiness.
  • Ensures that evidence-based standards of practice are used, and appropriate patient outcomes are met. 
  • Assesses and evaluates patient experience data and practices; leads improvement efforts. 
  • Establishes and maintains collaborative relationships with key external stakeholders to facilitate effective patient-centered care and operations.
  • Hires and supervises qualified staff; manages personnel in accordance with UWHCD’s policy.
  • Ensures an acting Administrator is available during all operating hours.
  • Successfully completes initial and ongoing organizational and department education, training, and competency requirements.
  • Infection Control: Understands, demonstrates, and follows organizational infection control guidelines (CDIC).
  • Quality: Focuses on improving processes and champions efficiency in daily work activities.
  • Safety: Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization’s occurrence reporting system and participating in developing improved safety processes, as appropriate. (ADM015, ADM016, OPS044, Mission Statement).
  • Performs other duties as assigned.


JOB REQUIREMENTS 

Licenses:

  • Valid driver’s license in the state of Wisconsin
  • State of Wisconsin professional licensure in Nursing, Physical Therapy, or another clinical field strongly preferred

Education:

  • Bachelor’s degree in clinical field (e.g., Nursing), health science, health care administration, public health, or related field required.

Experience:

  • Minimum of 5 years of recent experience in home or community-based care.
  • Experience and knowledge in evidence-based strategies to address clinical care, program outcomes, cost-effectiveness, and patient/family experience required.

Miscellaneous:

  • On-call responsibility required
  • Specific job skills required:
    • Demonstrated ability to seek out, analyze, and solve problems using data.
    • Excellent interpersonal, written, and verbal communication skills.
    • Promotes teamwork and is a strong collaborator with internal and external cross-functional teams.
    • Demonstrated creativity and flexibility.
    • Strong change agent.
  • Certifications in clinical, leadership, or improvement desired (e.g., OASIS, Home Health Certified Nurse, LSS).
