How you’ll contribute

An Outcomes Manager who excels in this role:

Coordinate PPD data collection with the interdisciplinary team.
Ensure supportive documentation for QIs is complete in the medical record.
Complete admission and discharge IRF-PAIs within CMS-designated timeframes.
Abstract required information from medical records and summarize on the IRF-PAI.
Enter data into the CMS-accepted software/program within designated timeframes.
Maintain and update the IRF-PAI log; verify accuracy against the patient location roster.

What we’re looking for

Applicants should have a clinical degree with current license to practice as an OT, PT, RN, SLP, or MSW Additional requirements include:

QI competency required
Working knowledge of state and federal guidelines; familiarity with healthcare workflows and documentation required
Basic computer and data entry proficiency required
Strong clinical assessment/analysis, effective verbal and written communication, excellent interpersonal skills, and ability to collaborate across disciplines
