The navigator is the liaison for Breast Center patients in need of multiple services and support. The Breast Nurse Navigator鈥檚 primary function is guiding breast health patients though the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. Perform the duties, responsibilities, and primary functions of a Registered Nurse in assessing, implementing, and evaluating the care of assigned patients. Be able to provide care in a variety of situations using solid judgment and critical thinking skills in making decisions. Work as part of a team to promote productivity and a positive working environment.

ESSENTIAL FUNCTIONS/PERFORMANCE EXPECTATIONS
路Complete a comprehensive assessment. This includes the initial and ongoing, extensive collection, analysis and interpretation of data.
路Synthesize the data collected during the comprehensive assessment to identify problems, make nursing diagnoses, and to formulate goals, teaching plans and outcomes.
路Implement the plan of care. The RN may begin, deliver, assign or delegate certain interventions within the plan of care for patients within legal, ethical, and regulatory parameters and in consideration of health restoration, disease prevention, wellness, and promotion of healthy lifestyles.
路Evaluate and report patient outcomes and responses to therapeutic interventions in comparison to benchmarks from evidence-based practice and research findings, and plans any follow-up care and referrals to appropriate resources that may be needed.

EDUCATION AND EXPERIENCE
路Bachelor of Science (BSN) Degree in Nursing required. If nurse does not have a BSN upon hire, the nurse must commit to enter a BSN program within two years of employment and complete the BSN program within five years.
路Current State of Texas License as a Registered Nurse
路Certification in area of specialty preferred
路ACLS required within 12 months of date of employment.
路Complete a minimum of 10 hours of education and/or training focused on care and treatment of the breast cancer patient within 12 months of hire.
路Maintain a minimum of 8 hours annually of education and/or training focused on the care and treatment of the breast cancer patient.
