About the Role
As a THL Care Coordinator, you will provide therapeutic interventions, case coordination, and resource linkage to clients and other involved parties (i.e., family, DCS, Court personnel, non-relative supports, etc.) to support identified changes within the individual or family. This role focuses on delivering client-centered services and collaborating with stakeholders to improve outcomes for children, adolescents, and families.

Responsibilities
· Provide therapeutic interventions to children and adolescents in a community-based treatment environment
· Coordinate care and services for assigned clients
· Link clients and families to appropriate community resources
· Collaborate with family members, DCS, court personnel, and non-relative supports
· Support identified behavioral and emotional goals for clients
· Maintain accurate and timely documentation in accordance with organizational and regulatory standards
· Participate in team meetings and case reviews as required
· Ensure services are delivered in a competent, ethical, and client-centered manner
· Provide therapeutic interventions to children and adolescents in a community-based treatment environment
· Coordinate care and services for assigned clients

Required Qualifications
· Bachelor’s degree in a Human Service discipline from an accredited four-year college or university
· Minimum of 1 year of experience working with children and adolescents in a therapeutic, community-based treatment environment
