Our Youth in Transition (YIT) School supports students with intellectual, emotional, and behavioral disabilities through compassionate, individualized, and student-centered educational services. We are seeking a collaborative and organized IEP/Transition Coordinator to support students in preparing for life after high school through individualized educational planning, transition services, and strong interdisciplinary collaboration. This role is ideal for an educator or special education professional who is passionate about advocacy, compliance, student growth, and helping young people successfully transition into adulthood, vocational opportunities, and greater independence. Ensure compliance with federal, state, and local special education regulations. Partner with families, school systems, and multidisciplinary teams to support student achievement. Train and support educators in effective special education practices and IEP implementation. Coordinate transition services and individualized supports for students across Diploma and Certificate Track programs.
