OPTIM MEDICAL CENTER SCREVEN is seeking a full time Physician Assistant or Nurse Practitioner for our location in Millen, GA.


Primary Duties: Job Title: Physician Assistant or Nurse Practitioner - RHC

Department: Rural Health Clinic

Reports To: Regional Administrator / Supervising Physician

Job Summary:
Provides comprehensive primary care services to patients in a rural health clinic setting under the supervision of a licensed physician and in accordance with applicable state laws and organizational policies. Evaluates, diagnoses, and treats acute and chronic medical conditions, promotes preventive healthcare, and collaborates with the healthcare team to ensure high-quality, patient-centered care for individuals across the lifespan.

Essential Duties and Responsibilities:
Patient Care
ā— Conduct comprehensive patient assessments, including medical histories and physical examinations.
ā— Diagnose and treat acute illnesses, chronic diseases, and minor injuries.
ā— Develop and implement treatment plans based on evidence-based practices.
ā— Order, interpret, and evaluate laboratory tests, diagnostic studies, and imaging results.
ā— Prescribe medications and treatments in accordance with state regulations and organizational policies.
ā— Perform minor office procedures within the scope of practice.
ā— Provide preventive care services, including annual wellness visits, immunizations, and health screenings.
ā— Educate patients and families regarding disease management, treatment plans, medications, and healthy lifestyle practices.

Care Coordination
ā— Collaborate with physicians, nurses, specialists, and ancillary staff to ensure continuity of care.
ā— Refer patients to specialists and community resources as appropriate.
ā— Participate in chronic care management and population health initiatives.
ā— Follow up on test results, referrals, and patient care plans.

Documentation and Compliance
ā— Accurately document patient encounters in the electronic medical record (EMR).
ā— Complete medical records, orders, and other documentation in a timely manner.
ā— Maintain compliance with Rural Health Clinic (RHC) regulations, HIPAA requirements, accreditation standards, and organizational policies.
ā— Participate in quality improvement activities and performance initiatives.

Organizational Responsibilities
ā— Support the mission, vision, and values of the organization.
ā— Participate in staff meetings, provider meetings, and continuing education activities.
ā— Assist with community outreach and health education programs as needed.
ā— Maintain professional licensure, certification, and continuing education requirements.

Qualifications:


Education & Experience:
Required
ā— Graduate of an accredited Physician Assistant or Nurse Practitioner program.
ā— Current Georgia Physician Assistant license or Current state registered nurse license/Current state license as an advanced registered nurse practitioner.
ā— Current DEA registration (or ability to obtain).
ā— Current Basic Life Support (BLS) certification.

Preferred
ā— One (1) to three (3) years of primary care, family practice, rural health, or community health experience.
ā— Advanced Cardiac Life Support (ACLS) certification.

Knowledge, Skills & Abilities:
ā— Strong clinical assessment and diagnostic skills.
ā— Knowledge of primary care, family medicine, and preventive health practices.
ā— Ability to manage patients across all age groups.
ā— Excellent communication and interpersonal skills.
ā— Ability to work independently while collaborating effectively with a healthcare team.
ā— Strong organizational and time management skills.
ā— Proficiency with electronic medical records and clinical documentation systems.
ā— Understanding of Rural Health Clinic regulations and reimbursement requirements preferred.

Physical Requirements:
ā— Ability to stand, walk, sit, bend, and reach throughout the workday.
ā— Ability to lift and carry up to 25 pounds occasionally.
ā— Manual dexterity sufficient to perform clinical procedures.
ā— Ability to work in a fast-paced healthcare environment.
