Community Care is hiring a Case Manager/Care Manager Day Shift for our Family Care Program in Milwaukee, WI. Offering an $8,000 sign-on bonus!聽This is a Monday through Friday position, 8:00 a.m. to 4:30 p.m.

Case Manager/Care Manager Job Responsibilities:

聽聽聽聽聽聽聽聽 Manage caseload of Family Care Program members聽聽

聽聽聽聽聽聽聽聽 Complete comprehensive clinical assessments聽

聽聽聽聽聽聽聽聽 Working collaboratively to develop, member centered care plans, provide on-going care coordination, member education, quality assurance and monitoring of services.聽聽

聽聽聽聽聽聽聽聽 Travel,聽Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.聽

Case Manager/Care Manager Job Requirements:

聽聽聽聽聽聽聽聽 Education:鈥疊achelor's Degree in Human Services or closely related field required.

聽聽聽聽聽聽聽聽 Licensure: Current long-term care functional screener certification preferred (eligibility for this certification is required)聽

聽聽聽聽聽聽聽聽 Experience: Minimum of 3 years recent experience with older adults or persons with physical or intellectual disabilities is required. 3 years of work in community nursing preferred.聽
