The Residential RN Case Manager provides nursing care and services for our residents. This is a part-time opportunity on our day shift for our Sacred Heart location at 1555 S. Layton Blvd. Milwaukee, WI 53215. 聽32 hours per week. Flexible schedule during normal business hours Monday-Friday. No holiday/weekends. No on-call responsibility. This position offer a $4,000 sign-on bonus.

Residential RN Case Manager Job聽Responsibilities:聽聽
路聽聽聽聽聽聽聽Work in collaboration鈥痺ith the Site Management Team to establish and maintain uniform, consistent care delivery for Community Care members, including collaborative problem solving related to member appointments, transportation, and home care needs.聽聽
路聽聽聽聽聽聽聽Conducts phone and in-person assessments using decision tools and resources to identify immediate and potential health needs.聽聽
路聽聽聽聽聽聽聽Completes all required documentation and charting and assists in the development and maintenance of member records.
聽
Residential RN Case Manager Job Requirements:聽聽
路聽聽聽聽聽聽聽Education:鈥疉ssociates degree in Nursing required.
路聽聽聽聽聽聽聽Experience: A minimum 2 years of personal, clinical leadership, and or employment experience providing care or service for a frail or elderly population required. 2 years of recent case management preferred. Experience in a long-term care setting preferred.
路聽聽聽聽聽聽聽Licensure:聽A current Wisconsin聽Registered Nurse license in good standing聽
路聽聽聽聽聽聽聽Certification:聽Current CPR/AED certification聽

路聽聽聽聽聽聽聽8:00 am to 4:30 pm, 32 hours a week 聽
路聽聽聽聽聽聽聽On call rotation
路聽聽聽聽聽聽聽No nights, weekends or holidays required
