Utilizing the case management process, the person in this position is responsible for providing care coordination services to a group of complex, special needs pediatric patients and their families across the continuum of care.

The RN case manager assesses, plans, implements, coordinates and evaluates the plan of care in partnership with the family and other members of the health care team.

• Full time
• Monday-Friday (8:00 am – 4:30 pm)
• No evenings or weekends
• Onsite at the Milwaukee campus

Education:

• Associate Degree in Nursing – required
• Bachelor’s Degree in Nursing – preferred

Skills & Experience:

• 2-3 years of experience with complex, chronic pediatric patients.

License requirement:

• License RN-Registered Nurse (30) - State of Wisconsin
• BLS-Basic Life Support Healthcare Provider - American Heart Association/American Red Cross/National Safety Council
