The Customer Transitions Coordinator serves as the assigned project manager for Consonus Pharmacy’s new customer onboardings, guiding senior living communities and long-term care facilities through the transition from their current pharmacy to Consonus. This role owns the end-to-end project plan for each onboarding — building detailed timelines, driving milestones and readiness activities, coordinating cross-functional teams, and serving as the primary customer-facing point of contact from sales handoff through go-live and post-start stabilization.

This position is outward-facing and project-oriented: it leads customer communication, facilitates kickoff and weekly transition calls, and manages the many moving pieces of each implementation — including intake, system build, integration/eMAR coordination, order-entry readiness, equipment logistics, and go-live readiness. The Customer Transitions Coordinator partners closely with internal and external stakeholders to deliver successful, on-time transitions that advance patient safety, operational excellence, and customer satisfaction.
