Qualifications and Requirements:

• Assessment of psychological, emotional and/or economic factors affecting patient's limitations and potential for a lack of improvement
• Collaborates with the physician and other members of the health team to develop a Plan of Care
• Master's Degree in Social Work is required
• At least 1 year of social work experience in the healthcare field preferred
• Comply with all aspects of the company's compliance and HIPPA regulations
• Must possess a valid driver's license
