As a home care Physical Therapist, you will be an integral member of a multi-disciplinary team that provides rehabilitative care to clients, allowing them to live independently and age in place safely. At CareAparent you will work with other passionate individuals focused on giving the best care possible for our clients. Our Care Management Model ensures that all team members work together to service our clients. Teams meet on a regular basis to answer questions, provide support, and stay connected.
Salary Range: $90,000.00 - $125,000.00

Responsibilities:
- Evaluate and treat clients suffering from physical disability due to injury, disease, or surgery in a home care environmentÂ
- Provide treatment for clients using a variety of methods including exercise, hands-on therapy, stretching maneuvers, and equipment to ease clients' pain and help them increase their mobilityÂ
- Test, measure, and assess clients' current condition and abilitiesÂ
- Assist patients with all phases of Physical Therapy treatment and techniques to include exercise instruction, manual therapy techniques, activities of daily living, functional/total body progressions, modalities, and a fall prevention programÂ
- Establish treatment plans specific to the client's specific needs that allows for excellent client outcomesÂ
- Make recommendations for devices or tools that might improve the client's quality of lifeÂ
- Work cooperatively and consistently with physicians, case managers, nurses, occupational therapists, and caregiversÂ
- Provide training to team members as neededÂ
- Request interventions to help avoid unnecessary trips to the emergency room or readmissionsÂ
- Monitor, document, and report clients' progress
Position Qualifications:
- Current Minnesota Physical Therapist license
- Graduation from an accredited physical therapy program as indicated by school transcript or diploma.Â
- Home care experience preferredÂ
- Ability to work independently and manage time effectively.Â
- Strong interpersonal skills, organizational skills, and problem-solving skills.Â
- Solid computer skills: prior experience with electronic medical records (EMR) preferred.
