What You’ll Do as a Physical Therapist

You’ll help patients maximize their independence, mobility, and quality of life — right in their own homes. Responsibilities include:

Delivering direct physical therapy services with excellence
Evaluating functional needs and updating care plans accordingly
Designing customized, outcome-based therapy plans
Collaborating with family members and care teams
Guiding and supporting therapy assistants
Maintaining accurate documentation, goals, and progress notes
Acting as a community and clinical resource
Working in a physically active, fast-paced care environment
Safely lifting, transferring, and assisting patients as needed

✔️ What You’ll Need

Graduate of an APTA-accredited PT program
Unrestricted Indiana PT license
1+ year of healthcare experience
Reliable vehicle, driver’s license, and current auto insurance
Ability to travel within the Mishawaka service region (up to 50%)
Ability to lift 50–100 lbs and meet physical demands

Reporting To:

Branch Director, Clinical Manager, Clinical Supervisor, or designee.
