What you will do:

Provides the exercise prescriptions and consultations, coordinates exercise functions for the program.  Conducts the wellness classes and presentations.  Reports to department manager.

Key Responsibilities:

  • Uphold the organization’s mission, core values, and compliance with all health, safety, and medical regulations.

  • Review patient evaluations and exercise histories to develop and update individualized exercise prescriptions in collaboration with healthcare providers.

  • Monitor patient health and safety during sessions, document progress, and adjust plans as needed.

  • Educate patients and families on disease processes, home exercise programs, stress management, and behavior modification.

  • Lead group exercise, relaxation, and educational sessions; assess patient responses and readiness for change.

  • Participate in discharge planning, team meetings, process improvement, and ongoing program development.

  • Maintain required competencies, certifications, and organizational standards.

What you will need:

  • Bachelor’s degree in physical therapy, Allied Health or Exercise Science. Certified by ACSM as Exercise Specialist or master's degree in Exercise/Exercise Physiology.

  • Current license to practice in the state of Indiana (if physical therapist).  Certified in Basic Life Support. ACLS Certification preferred.

  • Prefer two years previous experience in adult fitness and/or cardiac rehabilitation programs.
