Description:

This is a PRN position which will require on call shifts for psych assessments in the ED after hours/weekends and inpatient floors as needed.

Location: Mishawaka

Status: PRN - on call shifts

** Must be willing to work full time or part-time hours for training during orientation period.

What You Will Do

  • Manage a social work caseload, supporting patients and families experiencing social, emotional, or environmental challenges that impact their health

  • Collaborate closely with physicians, nursing staff, hospital personnel, and community agencies to ensure coordinated, high‑quality care

  • Participate in case conferences to assess social needs, family dynamics, strengths, and barriers to care

  • Advocate for patients by connecting them to community resources and supporting transitions across care settings

  • Help patients and families understand and follow medical recommendations to support optimal health outcomes

  • Maintain accurate clinical documentation and provide timely progress updates to physicians and care teams

  • Serve as a resource and educator to clinical staff on how emotional and social factors affect physical health

  • Build and maintain relationships with external clinicians and community partners to meet patient needs

  • Actively promote excellent customer service, patient safety, and organizational values

What You Will Need

  • Master’s degree in Social Work (MSW)

  • 1-2 years recent social work experience with Psych background strongly preferred.

  • Active and valid Indiana social work license

  • Minimum of one year of social work experience in a medical, healthcare, or mental health setting

  • Strong clinical judgment, critical thinking, and problem‑solving skills

  • Ability to communicate clearly and effectively with patients, families, clinicians, and community partners

  • Comfort working collaboratively in a multidisciplinary healthcare environment

  • Basic competency with electronic medical records and computer applications
