Saad Hospice is seeking an LPN who is looking for a change of pace in an office setting.

The position entails working in the office, answering and triaging calls from patients, caregivers, and employees.  


Responsible for helping with the below:


DUTIES AND RESPONSIBILITIES: 



  • Responsible for reviewing and documenting on the communication log daily and following up with resolutions added into the log. 

  • Answers after-hours phone calls, triaging symptoms via phone, communicating with the on-call team to assist with what is best needed for the Hospice patient.

  • Act as a resource for patient education and information, instructing patients/caregivers concerning treatments, test results, and other information as needed.

  •  In coordination with the Clinical Director, they determine the urgency of seeing the patient based on brief assessment and on familiarity with a patient's condition. 

  • Able to display professionalism when triaging emergent situations. 

  • Coordinates with the holistic care team to facilitate efficiency of services. 

  • Understands the needs of the organization and supports the mission, values, and management of patient care services. 

  • Assists the Scheduling Coordinator with the Scheduling Duties assigned.

  • Pulls and logs IPU supplies as needed. 

  • Helps coordinate transfer of a patient from the field to our inpatient unit for symptom management if necessary. 

  • Helps the on-call team get visits scheduled as needed and assists in communicating with patients/families.



Salary compensable with experience.

Schedule: 8p-8a 3 to 4 day weekly rotation with every other weekend rotation.
