Job DetailsLevel: ExperiencedJob Location: Center for Human Services - Modesto, CA 95350Position Type: Full TimeEducation Level: Masters Degree or higherSalary Range: $44.00 - $46.00 HourlyJob Shift: DayPosition Summary: The Licensed Clinician provides clinical and operational oversight for Enhanced Care Management (ECM), Community Supports, Wellness Coaches, and Community Health Worker programs. This role ensures that all services are person-centered, trauma-informed, culturally responsive, and compliant with DHCS, Medi-Cal, and Managed Care Plan standards. The Licensed Clinician provides direct supervision, clinical consultation, and quality assurance for identified program staff.    DUTIES:   Clinical Oversight & Care Planning   Ensure assessments and individualized care plans are completed in accordance with field guidelines.   Review, provide feedback on, and approve/sign off on assessments, intakes, care plans, and progress notes.   Provide clinical oversight for complex or high-risk cases.   Monitor documentation for accuracy, quality, and compliance with program and funder standards.   Support for ECM, Community Supports, Wellness Coaches, and Community Health Workers   Provide clinical guidance and support to staff implementing ECM, Community Supports, and wellness services.   Participate in Multi-Disciplinary Team (MDT) meetings to promote coordinated, integrated care.   Monitor ongoing care coordination and progress toward care plan goals.   Ensure that clinical oversight and supervision are scaled appropriately to program caseloads and service needs. Provide consultation and supervision consistent with licensing board requirements, including individual and/or group supervision for Wellness Coaches and Community Health Workers as applicable.   Team Support & Training   Provide debriefing and consultation for staff in difficult or high-risk situations.   Develop and deliver trainings on trauma-informed care, motivational interviewing, care coordination, cultural responsiveness, and other relevant topics.   Support team members in applying best practices and maintaining compliance with funder and regulatory expectations.     Clinical Resource & Care Coordination   Serve as a clinical resource to care team members, offering expertise on behavioral health, care planning, and member engagement.   Support medication reconciliation and care transitions as needed.   Collaborate with primary care, behavioral health, housing, and community partners to ensure holistic, person-centered care.   Facilitate coordination with Managed Care Plans, schools, healthcare providers, and community agencies.   Program Development & Quality Improvement   Integrate evidence-based practices and continuous improvement strategies into service delivery.   Analyze program data and outcomes to identify strengths and opportunities for improvement.   Support compliance with all local, state, and federal regulations.   Participate in committees, cross-agency teams, and community partnerships to advance program quality and coordination of care.     QualificationsQUALIFICATIONS:   Master’s degree in behavioral health or related field (LCSW, LMFT, or LPCC required).   Active California clinical license in good standing.   Minimum of 2 years of clinical experience, including at least 1 year in care management, population health, or community behavioral health.   At least 1 year of supervisory or leadership experience.   Ability to pass agency-paid criminal justice screening, including fingerprinting.   Ability to pass agency-paid TB (Tuberculosis) screening.   Ability to pass agency-paid health screening and/or drug testing, if required.   Valid California driver’s license and proof of auto insurance coverage required.  Ability to pass agency-paid MVR clearance (no more than 2 points; under age 25 cannot have any points per insurance requirements).      REQUIREMENTS:   Represent the agency in a professional and competent manner.   Advocate for the best interests of the agency and clients we serve.   Establish and maintain effective working relationships with the public, co-workers, clients, supervisors and members of diverse cultural and linguistic backgrounds regardless of race, color, creed, religion, religion, gender, sexual orientation, gender identity or expression, national origin, age, ancestry, political affiliation, citizenship, disability, medical conditions, marital status, amnesty and military or veteran status.   Will promote and support a culturally and linguistically diverse workforce and be responsive to the population within our service area.   Maintain confidentiality and confidential information in accordance with legal standards and/or agency regulations.   Participate in assigned scheduled agency meetings, in-service training, conferences and other trainings as determined by the supervisor.  This includes serving as an agency representative at assigned community meetings.   Observance of assigned working hours and program appointments by demonstrating promptness and thorough preparation.   Performance of assigned duties with a positive attitude and in the spirit of teamwork, collaboration and cooperation.   Communicate effectively both orally and in writing.   Perform job duties in a safe manner to ensure a safe working environment for oneself and others.   Participates in and/or supports agency fund development activities and events.   Preparation of assigned reports, work records, statistical data, job performance evaluations, work plans, etc. in a timely manner.   CHS desires all leaders have competency in the following:   Systems Thinker – Seeks to understand the big picture.    Creative/Innovator – Can both embrace and bring forward new ideas.   Empathic and Relational Leader – Inclusive and empowering of others.   Embrace Emergence – Explorative, ability to embrace and anticipate the unexpected   Boundary Spanner – Connect people across boundaries, extend past familiar territory, community builder.
